Change of name can only be done by virtue of Marriage or Divorce. In this case the following documents are required:
a) Marriage Certificate; b) Consent Letter signed by the husband; c) Data page of the husband's Passport; d) Applicant's Valid Passport.
The Mission also effect change of name as a result of Divorce. The applicant is to provide the following:
a) Divorce Certificate; b) Proof of maiden name, e.g. Passport before marriage; c) Other valid ID Card bearing maiden name.
Change of name by deed poll is not acceptable.

To collect a First issue passport, you must bring: Collection Slip, and a copy of Long Birth Certificate.
In all cases, if you are collecting for someone else, in addition to aforementioned requirements, you must also provide a signed letter of authority, and a photo identification of yourself.

This is to notify that effective Tuesday, 22nd December, 2020, all Immigration and Consular Services at Nigeria High Commission, London will be suspended to the general public until further notice.
