Passport Reissue (After 10 years)

Dual Nationality
§Completion
of Form C1;
§Two
studio taken passport photograph, one to be endorsed
on the reverse side by the Recommender
who must be a Nigerian;

and copy of Long Birth Certificate;
§Production
of valid British passport (valid at least six months),
with photocopy of pages 1-5 and
pages 32/33 if EEC passport style;
§Proof
of parent's nationality;
§Submission
of Guarantor ( different from the
Recommender) form who must be a Nigerian. The Guarantor
must submit photocopy of the data page of his/her valid
Nigerian Passport;
§If
applicant is a Minor, letter of consent from parents
and photocopy the data page of their valid Nigerian
Passport. Passport. In case of single parenthood, show
proof.
§Payment
of £54 in Postal Orders (made payable to the Nigeria
High Commission, London) or by Credit/Debit card;
§Application
must be submitted in person or by post through special
delivery (including return postage);
(PAYMENT BY CASH OR CHEQUE IS NOT ACCEPTABLE).
Replacement Of Lost Passport
§Letter
to the High Commissioner stating the circumstances of
loss of passport;
§Completion
of one copy of form C1;
§Submission
of the photocopy of the data page of a valid Nigerian
Passport of the Guarantor including the Guarantor's
Form (P.21) duly Sworn before the Commissioner for Oaths
at the Nigeria High Commission;
§Submission
of a Police Report clearly stating loss of the Nigerian
Passport;
§Submission
of Birth Certificate or Sworn Declaration of Age duly
signed by Commissioner for Oaths of Nigeria High Commission;
§Submission
of a Sworn Declaration of the loss of Passport signed
by the Commissioner for Oaths of the Nigeria High Commission;
§Submission
of two studio-taken passport photographs (one of which
must be endorsed on the reverse side by the Recommender
who must be a Nigerian);
§Documentary
proof of profession (Letter from Employer or P60);
§Payment
of £166.00 in Postal Orders(made payable to the
Nigeria High Commission, London) or by Credit/Debit
Card;
§Submission
of proof of applicant’s residence status i.e.
letter from the Home office showing evidence of stay
in UK;
§Proof
of applicant Address in UK i.e. 2 utility bills
(e.g. Gas , Electricity ) or Bank Statement;
§Letter
from the academic institution (Students only).
§Ticket
Stub/ Airline letter showing evidence of trip to the UK (for
Non-Resident).
NOTE:
All applications for re-issue of lost passports must
be submittedat the counter. Applicants would
thereafter be required to
submit themselves for personal interviews.
This is however subject to satisfactory
scrutiny of their submissions.
The Guarantor must be a reputable
Nigerian with good knowledge of the applicant
All submissions must be made

Endorsement of child on parent's passport.
Completion of Form C.A.F;
Letter of consent from the father. In case of single
parenthood, show proof;
Photocopies of the pages of father’s passport showing
identity and passport validity;
Photocopies of the child’s long Birth Certificate
and original;
Two passport photographs;
Production of mother’s passport for the endorsement;
Payment of £9 in Postal Order (made payable to the
Nigeria High Commission, London) or by Credit/Debit;

Issue of passport to a child of five years and above (First
Issue)

Completion of Form C2 and a Data form;
§
Letter of consent from both parents.
In case of single parenthood, show proof;
§
Photocopies of the data page of a valid
Nigerian Passport;
§
Photocopy of the child’s long birth certificate;
§
Two studio-taken passport size photographs
of the applicant;

Payment of:
i.For
children whose names are previously endorsed on parent's
passport, £39 in postal order( made payable to the Nigeria
High Commission, London) or by Credit/Debit card.
OR
ii.For
children whose names are not yet endorsed on the parent's
passport, £54 in postal order (made payable to the Nigeria
High Commission, London) or by Credit/Debit card.
AND
iii.Production
of Guarantors Form (P. 21) sworn before the Commissioner
of Oaths of Nigeria High Commission;

Emergency Travel Certificate
As the name implies, Emergency
Travel Certificate is issued for emergency purposes
only, when and if the standard passport is lost. ETCs
are issued to facilitate applicants immediate departure
for Nigeria only.
§Police
Report stating the loss of a Nigerian Passport;
§
Passport photographs of the applicant;
§
Original and photocopy of the return
ticket to Nigeria;
§
Payment of the fee of £33.00 in crossed
Postal Order (made payable to the
Nigeria High Commission);
§
Completion of application forms;
§Submission
of photocopies of data page of a valid Nigerian Passport
of Guarantor or pages
32/33 if EEC passport;
